Summer Food Service Program

Serving free meals locally

for all children under the age of 18

Children do not have to be enrolled at the school corporations to receive free meals through the U.S. Department of Agriculture’s Summer Foods Program. Meals are the same for all children regardless of race, color, national origin, sex, age or disability. There will be no discrimination in the course of the meal service.  Lunch will not be served at any location July 4.

Students with Special Dietary Needs

Serving Students with Special Dietary Needs

The Nutrition Service staff at Fairfield Community Schools is committed to serving healthy and nutritious foods for all students. If a student has a special dietary need, we will provide alternate foods if requested with a physician signed completed Medical Statement (see link below) along with a specific food plan submitted to the school nurse. For more information, please read the "Guidelines for Accommodating Children with Special Dietary Needs" below.

If your student has a special dietary need, please complete Part 1 of the Medical Statement form below and have a recognized medical authority complete Part 2 and return to the school nurse. No substitutions can be made without the completed form signed by a recognized medical authority on file with a specific food plan.

A recognized medical authority includes: M.D., N.P., or P.A. with prescriptive authority.
