Serving Students with Special Dietary Needs
The Nutrition Service staff at Fairfield Community Schools is committed to serving healthy and nutritious foods for all students. If a student has a special dietary need, we will provide alternate foods if requested with a physician signed completed Medical Statement (see link below) along with a specific food plan submitted to the school nurse. For more information, please read the “Guidelines for Accommodating Children with Special Dietary Needs” below.
If your student has a special dietary need, please complete Part 1 of the Medical Statement form below and have a recognized medical authority complete Part 2 and return to the school nurse. No substitutions can be made without the completed form signed by a recognized medical authority on file with a specific food plan.
A recognized medical authority includes: M.D., N.P., or P.A. with prescriptive authority.
